Please fill out the information on this form completely. A street address and phone number must be included. Orders are shipped via courier and cannot be delivered to a post office box.
Orders are typically processed and shipped from our warehouse in New Jersey within 5 to 7 working days. Please allow an additional 2 to 5 days for transit.
Free Shipping on orders of $24.99 or more! A Shipping and Handling charge of $6.85 will be assessed for orders less than $24.99 (total excludes shipping charge). Contact Customer Service at 1-800-648-8114 for shipping charges to Hawaii, Alaska, and Canada. Express, Overnight, and Saturday Delivery services are available in some locations for an additional fee.
